Diabetes Mellitus is an quandary that can be best described as a collection of morphologic and metabolic sequelae to an underlying disorder which, itself, is largely manifested by abnormalities in insulin and carbohydrate homeostasis. While it is unarguably inherited, it is strongly influenced by environment, being more frequent in those with caloric affluence and decreasing during times of deprivation. Globally, November is observed as the Diabetes Awareness Month and this edition of Metabolic Health Digest presents interesting articles that ranges from genetic to environmental influences on individuals with Diabetes Mellitus.
One article highlights the incidence of hyperglycemia and adequate management via dietary and insulin regimens in the case of gestational diabetes. It reflects on the efficiency of Glyburide being equally potent as insulin in terms of diabetes management as well as its safety aspect – that there was no transfer of Glyburide from mother to fetus or vice-versa in full-term infants.
One of the articles suggests that Patients with Type 2 Diabetes Mellitus (T2DM) have a high probability of developing Chronic Kidney Disease or the advancement of the staging post the initiation of second-line therapy. Yet another article highlights that overweight and obese patients with T2DM are at high risk of developing advanced fibrosis, hence the preventive measure here should not be missed by clinicians.
Other than these, new updated studies including comparative study for risk of kidney failure in T1DM and T2DM adults, discussion on the importance of lifestyle intervention during the preconception stage in reducing intergenerational obesity and diabetes risk, the importance of assessing non-albuminuric kidney parameters, management guidelines for T1DM from ADA, concept, and consequences of diabesity, and the association between sleep duration and obesity are included.
Furthermore, this editorial also highlights a case study of Bullous Diabeticorum which is a rare Cutaneous Manifestation in Diabetic Patients. Since it is a spontaneous condition, the treatment protocols should focus on preventing the formation of an ulcer.
